I acknowledge that participation in this class involves physical activity and the potential risk of injury. I confirm that I am physically fit and have no medical conditions that would prevent safe participation.

I release The Playground Studios, including its officers and staff, from any liability for injury, accident, or illness arising during the class.

Booking, Cancellation, and Refund Policy

I understand that all class bookings are non-transferable, non-cancellable, and non-refundable, regardless of attendance, scheduling conflicts, or personal circumstances. Failure to attend a booked class does not entitle me to a refund, credit, or rescheduling.

For Minors

By submitting this form, my parent(s) or legal guardian(s) and I acknowledge that participation involves certain risks, including possible injuries and exposure to mature choreography. With full understanding of these risks, consent is given for the minor to participate.

Participation is voluntary and subject to applicable rules and standards of conduct. We release The Playground Studios and its directors, officers, employees, volunteers, agents, and affiliates from any claims or liability related to participation.

I understand and consent that photos and videos may be taken before, during, and after classes and may be used by The Playground Studios for sales, marketing, and social media purposes.

I authorize The Playground Studios to use my information, images, videos, and related materials for marketing, communication, and service improvement purposes.
